I think my wife's took between six months and a year of constant MSM use (internally only) before hers faded and didn't come back (at least not nearly as noticeable). She was not trying to address the small area of melasma that she had and it just happened as a "side-effect". I would think that lotion applied consistently to the area would do even better than internally if done consistently and long enough. But then again, not everyone gets the same results.

I didn't notice any change at all. melasma tends to 'fade' in the winter and darken in the summer when exposed to sunlight. I'm using sunscreen to avoid having a face that looks like a world map this summer!- but I can still see the melasma.
